An independent review of the attack on a consulate in Benghazi, Libya has led to resignations in the State Department.

That September 11th attack killed four Americans, including Ambassador J. Christopher Stevens.

The independent review, released Tuesday, cited "systemic failures and leadership and management deficiencies" at the State Department at the time of the attack.

Two senior State Department officials tell CNN that Assistant Secretary of Diplomatic Security Eric Boswell and two other officials have now resigned.

Despite this criticism, the review board did not single out any individual for discipline.
