The U-S Food and Drug Administration released a report with the findings Monday.
The Ameridose facility, where the safety and quality concerns were found, is managed by the same company that runs the now closed New England Compounding Center, the source of the tainted injectable steroids.
The outbreak is being blamed for 32 deaths and nearly 400 more cases of presumed fungal meningitis.
Inspectors reported that Ameridose was not testing their drugs for sterility or potency.
Ameridose said they were preparing a response for the F-D-A but in six years, they've had no instances of contaminated products.
