The Department of Transportation report was obtained by the Wall Street Journal.
The paper reports investigators found oil and gas giant Exxon delayed in shutting down all the valves to a broken pipeline in the river, allowing more oil to spill into the river.
The spill cost an estimated $135 million in cleanup.
The report found flood-induced erosion exposed the pipeline, causing it to break underneath the river.
